Regardless of whether you opt for a virtual or in-person appointment, you'll be consulting with a licensed physician who is authorized to recommend Medical Cannabis Cards. The information exchanged during your visit will be treated as part of your confidential medical record, and your doctor will only disclose details about your medical condition and your cannabis recommendation for verification purposes.

What do I need to bring or have available at the appointment?

To support your diagnosis, please ensure that you bring a valid Ohio or Michigan Driver's license, State of Ohio or Michigan Identification card, or an unexpired United States Passport. Additionally, it is important to bring any relevant medical documentation pertaining to one of the qualifying conditions. This may include recent medical records, a list of current prescriptions (if applicable), a physician's letter (optional), and any X-rays, MRI's, or other test results that may be relevant.

How much does a medical cannabis card cost?

A medical cannabis card cost $240 for a full year.  Renewals are $150.  There are veteran and industry discounts. Always look for specials on our social media sites. Along with the evaluation, you will receive a treatment plan and medical cannabis education to start the process. All follow up appointments are included.

How do I pay for my appointment?

To complete the registration process, a payment to the Board of Pharmacy is required as mandated by the state. You can make this payment through the OMMCP website using a credit or debit card, or by mailing a check or money order.

For the physician appointment, payment can be made using a credit or debit card during a virtual appointment or in person using cash or card payment.
